Patient's Rights in Yokota Dental Clinic
  1. Patients have a right to be treated under conditions where their personalities are respected in a warm-hearted clean environment.
  2. Patients have a right to request that the confidentiality of their medical care including all medical records and contents of interviews be maintained .
  3. Patients have a right to be fully informed of their treatment plan, treatment period, treatment cost, risk of adverse effect from the proposed treatment.
  4. Patients have a right to select the treatment method they desire.
  5. Patients have a right to view their medical documents and materials including medical records, X-ray photographs, and impressions and to ask the doctor to explain these documents and materials.
  6. Patients have a right to obtain a copy of their medical records if there is a clear purpose for obtaining a copy such as requesting a second opinion from another doctor.
  7. Patients have a right to be referred to another suitable hospital if our facilities cannot provide the required treatment or the patient has refused treatment in our clinic.
  8. Patients have a right to receive a detailed statement of treatment costs showing technical fees, examination fees, and drug expenses.
  9. Patients have a right to discontinue the treatment, to request further explanation of the treatment, and to ask the doctor to change the treatment even if the treatment is in progress.

Patients have a right to receive advice or emergency care from our doctor even outside regular office hours.
